Last week, Monod Bio, a spinout company from the University of Washington, announced that it has received a $1.5 million Gates Foundation grant to develop a urine-based rapid diagnostic test for tuberculosis (TB). The company aims to help fill the gap of non-sputum-based, rapid, point-of-care TB diagnostics that are suitable for low-resource settings. Most World Health Organization-recommended TB diagnostics rely on sputum, which not all patients can provide and can be logistically difficult for health care providers to collect, but current alternatives using urine and stool samples have low specificity, and current TB tests are also not available at the point of care in communities. The test will utilize the company’s NovoBodies, novel proteins designed to be highly specific and sensitive—diagnostics have primarily used natural proteins that are adapted for use in tests.  The Combating Antibiotic-Resistant Bacteria Biopharmaceutical Accelerator (CARB-X) last week announced that it is awarding $1 million to Zeteo Tech to further work on a rapid, non-invasive diagnostic test that can diagnose lower respiratory tract infections, like pneumonia, using breath samples. Lower respiratory tract infections kill 700,000 children under five annually. A less invasive, rapid test could help speed up diagnosis and guide more effective treatment, particularly of drug-resistant infections, helping reduce the prescribing of unnecessary antibiotics that can fuel the rise of antibiotic resistance. Zeteo’s platform analyzes the tiny particles and droplets in breath samples to distinguish between viral and bacterial infections and assess antibiotic susceptibility.  The results of a clinical trial released last week found that a common over-the-counter nasal spray that is used for seasonal allergies could work as an antiviral against a range of respiratory infections, including influenza, RSV, and the virus that causes COVID-19. The researchers suggest that the nasal spray, Azelastine, could help block COVID-19 infections by inhibiting a key enzyme that the virus uses to replicate or by preventing the virus from latching on to the point at which the virus enters human cells. If proven safe and effective in further studies, Azelastine could serve as an easily scalable, over-the-counter prevention method complementary to vaccines and other tools, especially when a virus is spreading among a community or in high-risk situations, like when traveling or attending crowded events. The research also provides another potential target for future vaccines.

It is important to continue advancing the vaccine’s development in between outbreaks, coordinating with partners to generate key data, accelerate and improve manufacturing for the later stages of development and production, and ensure that a trial can begin promptly following the identification of a new outbreak. IAVI’s vaccine was tested in a trial during Uganda’s Sudan virus outbreak earlier this year and previously demonstrated promising results in animal models and a Phase 1 clinical trial in the United States.Researchers at the University of Texas and Moderna recently announced that in a preclinical trial in rodents, their mRNA vaccine candidate provided full protection against Marburg disease, a deadly disease closely related to Ebola, for which there are currently no licensed vaccines or treatments. Marburg has caused several outbreaks in sub-Saharan Africa, including one in Tanzania earlier this year, and the frequency of outbreaks has increased in recent years, as behavioral shifts offer more opportunities for the virus to spill over from fruit bats to humans. If the vaccine proves successful in future human trials, it could help play a key role in curbing the spread of future outbreaks.Last week, Novartis announced that the first malaria treatment for babies and small children who weigh less than 10 pounds has been approved by Swiss authorities, offering a new tool in the fight against malaria, particularly to help reduce the high rate of malaria deaths in young children in sub-Saharan Africa. Coartem Baby, developed by Novartis and the Medicines for Malaria Venture, offers significant improvement over the previous option of treating these babies and young children with versions of malaria drugs formulated for older children, which raises the risk of toxicity and overdose. Novartis plans to introduce the drug on a largely not-for-profit basis, and it could be rolled out in African countries within weeks, with the eight countries that participated in clinical trials of the drug expected to be among the first to receive it.

The systematic review looked at 34 journal articles with data from 8,000 babies under nine months old in low- and middle-income countries, finding that maternal measles antibody rates were highest at birth before decreasing rapidly, leaving infants with low antibody rates for months before their first recommended measles vaccine dose at 9-12 months, followed by a second at 15-18 months. An earlier first dose, achieved either by giving the first dose sooner or adding a third dose before the routine two-dose schedule, could help close that critical gap in immunity.Last week, Roche announced that it is launching a Phase 3 trial of its novel antibiotic candidate, zosurabalpin, in patients with carbapenem-resistant Acinetobacter baumannii (CRAB) infections, considered one of the most dangerous and deadly antibiotic-resistant pathogens and a priority for antibiotic development. The study, expected to start at the end of 2025 or early 2026, will compare the safety and efficacy of zosurabalpin with standard-of-care antibiotics in around 400 patients with CRAB infections at sites across Europe, North and South America, and Asia. Zosurabalpin was previously shown to demonstrate high efficacy in lab tests against human clinical CRAB isolates and in mice with lung and thigh infections caused by pan–drug-resistant A baumannii.The Coalition for Epidemic Preparedness (CEPI) announced last week that it has signed a new agreement with the US Department of Defense’s Joint Program Executive Office for Chemical, Biological, Radiological and Nuclear Defense to enable collaboration on several projects to boost global preparedness against future disease outbreaks, including through the development of medical countermeasures. The first project will support the development of a monoclonal antibody for the Nipah virus, a deadly zoonotic disease with no approved tools to defend against it. The department will transfer doses of a Nipah monoclonal antibody, which is currently undergoing Phase 1 testing in the United States, to CEPI for a Phase 1b/2a clinical trial in India and Bangladesh, two countries that see almost annual Nipah outbreaks. These trials aim to provide critical data that could enable access to the monoclonal antibody to protect vulnerable populations during an outbreak in Nipah-affected regions and the US joint forces in a potential emergency.

The new study fills a gap in knowledge of the safety and efficacy of this approach for young children, who face the highest burden of clinical malaria and are particularly at risk of artemisinin-resistant strains but could face adverse complications from adult formulations of primaquine. The findings highlight the need for child-friendly formulations of primaquine and support the expanded use of the drug in regions with a moderate-to-high transmission rate and the threat of artemisinin partial resistance.The Drugs for Neglected Diseases initiative, the National Health Service University Hospitals of Liverpool Group, and the University of Liverpool have initiated a clinical trial in Liverpool, United Kingdom to test an oral drug for visceral leishmaniasis, one of the world’s deadliest parasitic diseases. Current treatments for visceral leishmaniasis, which is almost always fatal if left untreated, include daily injections for up to 17 days, which can cause immediate pain, as well as nausea, vomiting, and more serious complications. The new oral drug, DNDI-6899, is easier to store, transport, and administer than current treatments. If proven successful, the drug could help make treatment more accessible to those who need it most.DioSynVax and ACM BioLabs have signed a new deal to jointly develop a next-generation avian flu vaccine with the potential for mucosal vaccine delivery, as the virus continues its worrying spread. The mRNA vaccine targets all major clades of the H5 avian flu subtype and could be scaled for rapid distribution globally in the case of a pandemic. The vaccine is also designed to be delivered via nasal spray, which could help increase uptake; to induce mucosal immunity, which helps fight the pathogen before it starts circulating the body; and to be temperature stable, enabling rollout in lower-resource settings.
